Hey zb, From Sitefinity 12.1 there is an option for the users to update easily the system email templates. Here you can find more information: www.progress.com/.../manage-templates-for-system-emails Unfortunately we don't pass the form name value in the template dynamic data collection, we only pass the form title. So in this case you can't update the template with this information. If you need this we could do it easily for some 12.1 internal build. If you're running an old Sitefinity version the only option is to attach for IFormEntryEvent and to create your own logic for notifications. Hope you'll find this information useful. Regards, Stoyan
