system error on log file

getting the following errors in log file on 10.0b02.

We had similar a couple of weeks ago but on a different table on a different index. Essentially this caused the database to not want to re-start after a shutdown and we had to dump and load the data out (coming a cropper in a nice data dictionary bug with date-time fields along the way)

Any ideas how to investigate and resolve this?
