PDFInclude works very well. However, you mention that you need to fill out a template. I don't think PDFInclude supports that in this version, but they are working on that. Check out http://sourceforge.net/projects/pdf-inc for PDFInclude files, help and status.

Are you trying to build the Powertools with the MXP scripts? What error message do you get?
